Rebecca Welch was tasked with checking on the welfare of 31-year-old Amelia Rose each 10 minutes on the Millbrook Mental Health Unit in Sutton-in-Ashfield, Nottinghamshire, on 31 January 2020.

“We will never get to see the amazing person she would have become, a loving daughter, sister, aunt and possibly a mother. Our lives will never be the same again.”

Detective Inspector Claire Gibson said her thoughts were with Ms Rose's family.

He added: “Welch’s failure to carry out the mandatory inspections robbed us of any opportunity to prevent Amelia from taking these steps or to potentially intervene to save her life.”

“Creating false records in an effort to defend oneself is particularly reprehensible.”
